• I collect essential personal information for contact and identification purposes. During your appointments, notes will be taken to support the provision of therapy services and to meet legal obligations.

• All interactions, including appointments and communications, are logged securely & confidentially, with your therapy and personal information stored safely.

• Your information will remain confidential, except in cases where disclosure is necessary to ensure your safety or the safety of others, or to report criminal activity, as required by law. In most instances, we will discuss any such circumstances during a session and agree on an appropriate course of action together.

• If you have not consented to communication with your GP, this may be overridden in the situations described above.

• I also ask that you respect the confidentiality of the therapy sessions. If you wish to record your sessions, please discuss this with me first so that I can provide written consent.  

• Please keep me updated with any changes to your contact details to ensure records remain accurate.

• Under data protection laws, you have the right to verify the accuracy of the information held, request updates, corrections, deletions, or additional information, and obtain a copy of your personal data. To request access to your information, please contact me using the details provided in this agreement.

• In accordance with GDPR, your records will be securely retained for 7 years after the end of treatment to comply with legal requirements. After this period, your information will be securely deleted. 

• Please note that before commencing sessions we will discuss privacy and confidentiality and you will be required to sign a consent form. I am happy to answer any questions you may have.
